SPONSOR FORM

TASK 1: Your disco dancers need to collect sponsorship for their chosen charity and need a personalised sponsor form.  It must:

• be one 2 sides of A4
• include information about the chosen charity and some information about the dancer if you wish
  
• indicate the time slots when this dancer will be taking part
• be printable
  
• give instructions to the dancer about what to do with the sponsor money.
